Insurance fraud is no longer a narrow back-office leakage problem. In the United States, the Coalition Against Insurance Fraud says fraud costs consumers more than $308.6 billion each year; insurance fraud costs U.S. consumers approximately $308.6 billion annually, and the annual value of insurance fraud is estimated to approach $80 billion, significantly impacting the costs for consumers and the insurance industry. These losses are measured in billions of dollars and directly affect the typical family’s premiums. The National Association of Insurance Commissioners also points to the Federal Bureau of Investigation estimate that fraud adds roughly $400 to $700 a year to the typical family’s premiums; and the National Insurance Crime Bureau warns that identity-driven schemes are increasingly feeding broader insurance crime, with insurance companies playing a critical role in combating these losses. Against that backdrop, AI has moved from “nice to have” to operational necessity. It gives carriers a way to inspect claims, documents, and identities at machine speed while reserving human expertise for the cases that truly deserve it.

The fraud problem is getting more expensive

The headline number matters because it reframes fraud as a margin issue, not just a compliance issue. Insurance fraud can be classified into two main categories: hard fraud and soft fraud. Fraud occurs in about 10% of property-casualty losses according to the coalition’s statistics page, and the regulator association’s insurance-fraud overview breaks the burden out across life, health, workers’ compensation, and property and casualty lines. Even if insurers debate the exact figure by segment or jurisdiction, the broad conclusion is hard to escape: insurance fraud is a multi-billion-dollar drag on claims costs, premium affordability, and operational capacity.

The underlying schemes are familiar, but the execution has become sharper. Staged crashes remain a live problem; the crime bureau’s examples show how organized groups can manufacture apparently plausible losses and injury narratives. Hard fraud involves deliberately planning or inventing a loss, such as a staged accident or arson, to claim insurance benefits, often through coordinated efforts by groups or criminal rings. In contrast, soft fraud, also known as opportunistic fraud, occurs when policyholders exaggerate legitimate claims, such as inflating the extent of damages in an accident. Common forms of insurance fraud include filing claims for accidents that never occurred, misrepresenting facts on applications, and staging accidents. In the UK, it was estimated that 30,000 auto accidents were staged in 2009, highlighting the prevalence of organized schemes and groups that commit insurance fraud. How AI helps fight insurance fraud and stops common fraud scenarios

A fake repair invoice after a car accident is a classic example of how automobile insurance fraud occurs. To prevent false claims, it is important to document incidents by taking photos and counting passengers at the scene. The smartest starting point is rarely “replace the whole fraud function.” It is usually one narrow, painful workflow with measurable leakage: auto repair invoices, incoming medical attachments, duplicate claimant review, or first-notice-of-loss triage. From there, the work becomes practical. Clean the labels. Separate confirmed fraud from mere suspicion. Decide what matters most for each workflow: higher recall, fewer false positives, shorter cycle time, or more consistent escalation. Then put a human-in-the-loop structure around it. Recent supervisory guidance is explicit here: insurers need records, traceability, risk-based controls, bias management, calibration, and performance monitoring, especially where claims decisions affect customers directly.

Insurance fraud bureaus, fraud bureaus, and state fraud bureaus play a key role in investigating and combating insurance fraud, often collaborating with public groups and private and public groups to share information and implement anti-fraud strategies. Health and Human Services, including Medicare and broader human services programs, are also involved in anti-fraud efforts, working alongside these groups to prevent and detect fraud, especially in healthcare.
